Orchiectomy

Orchiectomy is a surgical procedure involving the removal of one or both testicles. It is performed for therapeutic purposes in various conditions and may also be part of hormonal therapy. The procedure aims to eliminate pathological processes or reduce androgen levels and is carried out in accordance with modern surgical and anesthetic standards.

Indications

• Malignant testicular tumors
• Suspicion of testicular tumor
• Hormone-dependent tumors (e.g., prostate cancer)
• Severe testicular trauma with non-viability
• Necrosis (including testicular torsion)
• Chronic inflammatory conditions resistant to treatment
• Gender-affirming surgical procedures

Procedure / Duration

The surgery is performed under local, regional, or general anesthesia. The surgeon makes an incision in the inguinal or scrotal area, followed by removal of the testicle with ligation of the vessels and spermatic cord. A testicular prosthesis may be inserted if needed. The procedure typically lasts 30 to 60 minutes.

Contraindications

• Severe general condition of the patient
• Decompensated cardiovascular diseases
• Acute infectious diseases
• Uncorrected coagulation disorders
• Severe respiratory insufficiency
• Exacerbation of chronic diseases

In oncological or emergency situations, absolute contraindications may be absent

Rehabilitation / Limitations

• Hospital stay usually 1–3 days
• Limitation of physical activity for 2–4 weeks
• Avoid heavy lifting and intense exercise
• Wearing supportive underwear (scrotal support)
• Proper postoperative wound care
• Sexual abstinence for 2–3 weeks
• Follow-up visit and, if needed, suture removal after 7–10 days

Hormone replacement therapy may be required after bilateral orchiectomy
