Glucose is a laboratory test that measures the amount of glucose excreted in urine over a 24-hour period. Normally, glucose is almost completely reabsorbed in the kidneys and is either absent or present only in trace amounts in urine. The presence or increase of glucose in urine may indicate disturbances in carbohydrate metabolism or impaired kidney function.

Indications

  • suspected diabetes mellitus
  • monitoring of diabetes mellitus
  • assessment of effectiveness of antidiabetic therapy
  • detection of carbohydrate metabolism disorders
  • suspected renal glycosuria
  • evaluation in cases of excessive thirst, weakness, or frequent urination
  • monitoring patients with endocrine disorders
  • general assessment of metabolic disorders

Procedure

  • the test is performed using a 24-hour urine collection
  • the first morning urine sample is not collected
  • afterward, all urine passed during 24 hours is collected in a clean container
  • the final portion is added the following morning at the same time the collection started
  • after completion it is necessary to:
    • measure and record the total urine volume
    • mix the container contents
    • transfer a small sample into a sterile container for laboratory testing
  • during collection, the container should be stored in a cool place

Interpretation of Results

  • Elevated urine glucose:
    • most commonly associated with diabetes mellitus
    • may occur with significantly elevated blood glucose levels
    • may result from a reduced renal threshold for glucose
    • sometimes associated with endocrine disorders
  • Absence or low levels:
    • considered normal
    • does not fully exclude early carbohydrate metabolism disorders

Clinical Significance

  • helps detect carbohydrate metabolism disorders
  • used for monitoring diabetes mellitus and treatment effectiveness
  • provides indirect information about blood glucose levels
  • important in diagnosing renal glycosuria
  • useful in the comprehensive evaluation of endocrine and metabolic disorders
