Anal skin tag excision (perianal skin fold removal) is a minor surgical procedure performed to remove excess skin located around the anal opening. Anal skin tags are benign skin growths that commonly develop after hemorrhoidal episodes, anal fissures, local inflammation, pregnancy, or chronic constipation. Although they are not malignant, they may cause discomfort, hygiene difficulties, irritation, itching, and repeated trauma to the area.

Indications

  • symptomatic anal skin tags;
  • difficulty maintaining perianal hygiene;
  • recurrent irritation, itching, or inflammation;
  • discomfort during sitting, walking, or wearing underwear;
  • repeated trauma or bleeding due to irritation;
  • need for diagnostic confirmation through histopathological examination in selected cases;
  • functional or cosmetic concerns following medical evaluation.

Procedure

The procedure is usually performed on an outpatient basis under local anesthesia. After preparing the treatment area, the physician removes the skin tag using appropriate surgical instruments. Depending on the size and location of the lesion, the wound may be left to heal naturally or closed with sutures. A dressing is applied at the end of the procedure, and the patient receives instructions regarding wound care and local hygiene.

Contraindications

  • active perianal infections;
  • severe acute inflammation in the treatment area;
  • uncontrolled bleeding or coagulation disorders;
  • medical conditions that significantly increase procedural risk;
  • lesions requiring further evaluation before removal.

The final decision is made by the treating specialist based on an individual clinical assessment.

Benefits

  • minimally invasive procedure;
  • performed on an outpatient basis;
  • short procedure time;
  • relief of discomfort, itching, and irritation;
  • improved perianal hygiene;
  • reduced risk of repeated trauma;
  • improved functional and cosmetic outcomes.

Limitations

  • the procedure does not prevent new skin tags from developing if underlying risk factors persist;
  • temporary local discomfort may occur during healing;
  • complete healing may take several weeks;
  • minor scarring or changes in local sensitivity may occur in some cases;
  • associated anorectal conditions may require additional treatment.

Preparation:

Patients should inform their physician about current medications, allergies, chronic illnesses, coagulation disorders, and previous proctologic procedures. A proctologic examination, including anoscopy or other investigations, may be recommended depending on the symptoms.

The perianal area should be kept clean before the procedure. In most cases, special bowel preparation is not required. Regular medications should not be discontinued without medical advice.
