Endoscopic paranasal sinus surgery (FESS — Functional Endoscopic Sinus Surgery) is a minimally invasive surgical procedure used in chronic polypous rhinosinusitis (CPRS). The procedure is performed through natural nasal passages without external incisions, using an endoscope with a camera and light source for real-time visualization of internal structures.

The surgery aims to remove polyps and inflamed mucosa, restore natural ventilation and sinus drainage, and reduce chronic inflammation. The procedure improves nasal breathing, sense of smell, and the effectiveness of subsequent medical treatment.

Indications

  • Chronic polypous rhinosinusitis resistant to medical therapy
  • Polyp recurrence after previous surgery
  • Severe nasal obstruction
  • Anosmia or hyposmia
  • Frequent or complicated sinusitis
  • Polyps causing pain or affecting adjacent structures
  • Benign or malignant tumors of the nasal cavity and paranasal sinuses
  • Associated anatomical variations (deviated septum, turbinate hypertrophy)

Procedure / Duration

The operation is performed endoscopically through the nasal cavity using high-resolution optical systems.

During surgery, polyps and inflamed tissues are removed, sinus ostia are widened, and natural drainage is restored. Specialized instruments, including a microdebrider, allow precise tissue removal with simultaneous aspiration.

Soft nasal packing may be applied for 24–48 hours if needed. The procedure typically lasts 60–120 minutes.

Contraindications

  • Severe general condition
  • Decompensated systemic diseases
  • Uncorrected coagulation disorders
  • Acute infections
  • Exacerbation of chronic conditions
  • Severe comorbidities limiting anesthesia

Rehabilitation / Limitations

  • Hospitalization up to 24 hours
  • Nasal packing (if required) for 24–48 hours
  • Recovery period 2–3 weeks
  • Regular nasal irrigation with isotonic solutions
  • Topical anti-inflammatory therapy (including intranasal corticosteroids)
  • Limitation of physical activity for 2–3 weeks
  • Avoid overheating (sauna, hot bath)
  • Regular ENT follow-up
